Patient Info

First Visit

Patients usually meet us for the first time during a pre-surgery consultation. During our meeting, we will spend time getting to know you, learning about the reason for your visit, and discussing your diagnosis and treatment options.

We will review current x-rays that you bring to your appointment. If you do not have current dental x-rays, we will take new ones while you’re in the office. Please come to your appointment with a list of questions and concerns. We will provide you with guidance, honest advice, and recommendations. If surgery is required, you will have the opportunity to schedule an appointment for a future date. Unless it’s an emergency, treatment is usually not planned for the same days as your initial meeting with the doctor.

Patient Registration

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Please bring the following items to your first appointment:

  • If you have them, x-rays and images from other dental offices. (You may request that your current/previous dentist forward them directly to our office.) Depending on your diagnosis, new images will be required for some patients.
  • List of current medications
  • Your medical and dental insurance card(s)

Financial Policy

We contract with many medical and dental insurance companies. Please contact us to verify that your insurance plan is on our list and to ask questions about your benefits. Feel free to call insurance provider directly. This helps confirm your coverage.

Please bring your insurance card(s) to your first appointment. We will give you a written estimate of your out-of-pocket financial responsibility based on your insurance coverage and deductible. The balance due, after insurance is applied, is your responsibility and is due on the day of your surgery.

We offer in-house payment plans and third-party financing through CareCredit®. We are happy to give you more information when you schedule your appointment. CareCredit® allows you to finance healthcare and dental care services. The card can be used to pay copayments, deductibles, and out-of-pocket fees for office visits, surgery, and more. Applications are available online. If you have questions, please let us know. We are happy to assist in any way we can. For your convenience, we also accept most major credit cards and cash payments.
